Our pledge regarding health information
We understand that health information about you is personal. We are committed to protecting it. We are required by law to maintain the privacy of protected health information ("PHI") and to provide individuals with notice of our legal duties and privacy practices.
How we may use and disclose PHI
We may use and disclose PHI for treatment, payment, and healthcare operations as permitted by HIPAA. Examples include sharing information with the physician treating you, with the pharmacy dispensing your medication, and with our business associates that perform services on our behalf under appropriate agreements.
Your rights
You have the right to inspect and obtain a copy of your PHI, request an amendment, request restrictions on certain uses and disclosures, receive confidential communications, and receive an accounting of disclosures.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
